This form needs to be filled out to have the applicant’s name changed on their birth certificate if they were born in Colorado. This is an important step in the process.

There is a $20 fee for having the state change a name on a birth certificate. You can also have the state send you copies of the new certificate for an additional fee. These are detailed in the form interview and you will have a chance to order copies from there.

When you come to the end of this interview you will be able to download and print out the birth certificate update form. There are instructions on the form for paying by credit card or mailing in a check.
  • This form is unique and is not filed with the Name Change Petition Packet.
It also cannot be filed until the name change is final with the court. For now, keep it on the side on its own and do not add it to any other forms.
  • Born in another state or country?
If the applicant was born in another state or country they cannot use this form. Please contact the vital records department in the applicant’s birth state or country to change the name on their birth certificate.
  • Can I change the gender on my birth certificate?
We can help you change the gender on your Colorado birth certificate.  Gender updates will create the Colorado Sex Designation Form as part of the birth certificate update packet.
  • If the gender change is for an adult over 18 – please do not sign the Sex Designation Form after its printed! You will need to sign section A of the form in front of a notary.
  • If the gender change is for a minor child under 18 - a doctor or mental health provider will need to fill out section B confirming they no longer identify with their birth gender. No medical treatment is required for this.

    For minors under 18 section A does not need to be filled out. Please leave this part of the form blank.
The interview will also create a short letter saying you no longer identify with your birth gender and want to change it. This letter needs to be signed by the person changing their name if it is for an adult. If the gender change is for a minor under 18, a parent or guardian will need to sign the letter for them.

The Colorado Sex Designation Form and the signed letter both need to be mailed in along with the birth certificate update form.
