Filing a Fee Waiver Form
If you are filing your Name Change Petition Packet in a county court there is an $88 filing fee. If you are filing your case in district court there is a $238 filing fee.
Please check the court name listed on the top left corner of your Name Change Petition Packet forms. This should tell you if you are filing in district or county court. Some names changes involving minors will need to be filed in district court.
If you are unable to pay the name change filing fee you may be eligible for the court to waive filing fees or offer a payment plan.

If you would like to ask the court to consider waiving filing fees please fill out the Filing Fee Waiver Form using the button below.
There is no guarantee that the court will waive your fees. If they are unable to waive your fees you can ask if they can do a payment plan when you file.
When you come to the end of this interview you will be able to download and print out the fee waiver form. You can sign this form right away. But do not sign any of the previous forms!
You will also need to gather the following support documents to file with your filing fee waiver:
Copies of bank statement for the past 3 months (savings and checking)
Copies of pay stubs or other proof of income for the past 3 months
If you receive unemployment benefits, social security, SSI, alimony, or workers compensation these all need to be provided as part of the proof of income above
Make sure you add this form and the support documents to your existing Name Change Petition Packet and keep them separate from the other form packets you fill out after this one.
