This is an optional step in the Colorado Name Change process.
If you are only changing the name on your Colorado driver’s license you do not need to complete this form.
If you are also changing the gender marker on your license you will need to fill it out.

If you would like to update the gender marker on your driver’s license please fill out the interview using the button below.
When you come to the end of this interview you will be able to download and print out the Change of Sex Designation form. You can sign and print it right away.
If the gender change is for an adult 18 or older, this is all you need to do. For an adult name change you can skip the next paragraph.
If the name change is for a minor child under 18, you will also need to have a medical or mental health care provider sign the form. This is just to confirm that the minor child no longer identifies with their birth gender. It does not require medical treatment of any kind.
This form is unique and is not filed with the Name Change Petition Packet. It also cannot be filed until the name change is final with the court. For now, keep it on the side on its own and do not add it to any other forms.
