This can be done at any time after a decree has been entered in your case.

Besides, you will have ready your JDF 1825 Order for Name Restoration After Dissolution of Marriage/Civil Union or Legal Separation along with JDF 1824. We fill only the information in the caption (that is the information to be filled out). The court will fill out the rest.
